This document field is for evidence of your 5 yearly GMC revalidation.
To approve the document, we need to see:
- Your name
- Your GMC number
- The date of your last or next revalidation meeting
This must be official documentation (for example a letter/email or screenshot) provided by the GMC or from your online GMC account.
