When you go to your 'Upload documents' page on your profile, you will see several different document fields for your immunisations.
These fields are:
- Occupational Health/Immunisations
- Hep B
- Varicella (Chicken Pox)
The 'Occupational Health/Immunisations' field is for a full list of the immunisations or blood tests you have received. This document would include the immunisations stated above, plus any others you may have received in your lifetime.
You can ask your own GP or practice to provide physical documentation of these records.
To approve a document in the 'Occupational Health/Immunisations' field, we would require to see that this states:
- Your name
- At least 5+ immunisations/blood tests
Alternatively, we can accept an occupational health letter, this must be an official document provided by your practice.
For the individual fields, e.g. Hep B, Varicella, you can upload any official documentation you hold which states these immunisations. Similarly, these letters or records must state your name.
It is acceptable to upload the same document to each field, providing it states the specific immunisation.
If you don't have your immunisation history documentation, you can request an Occupational Health assessment, which are blood tests carried out at your local GP practice or hospital. If this option is not available to you, please reach out to a private provider. You will receive a document with the results and this can be uploaded into the relevant field.
