Uploading documents
It is quick and easy for you to upload documents straight to your profile. You just need to have a clear photograph/scan of the document that you are uploading, ensuring no sections are cut off. Photos can be taken on a smartphone.
The three core documents needed are:
- CV
- Passport
- DBS certificate*

*Please note, the DBS certificate requires specific consent. Please tick the consent box when uploading it into the document field. If you are having trouble uploading the document and need assistance, we will need this consent in writing to be able to upload this document on your behalf.
Consent from a user is required for the following questions:
To comply with data protection regulations, we need your consent to share your DBS certificate with practices. If you decline, we cannot approve your profile. You will see the following options:
DBS Update Service Consent
If you subscribe to the Disclosure and Barring Service (DBS) Update Service, we request your consent, allowing Lantum and/or any prospective employing GP practice/organisation to check up-to-date information referring to your DBS certificate on the DBS Update Service.
