The purpose of exception reports is to ensure prompt resolution and / or remedial action to ensure that safe working hours are maintained. Exception reporting is the mechanism used by doctors to ensure compensation for all work performed and uphold agreed educational opportunities.
To Submit a new Exception Report
- Click on Home icon and select Submit Exception Report.
- Click on the Calendar Icon to choose the date the exception took place.
- Dropdown of the calendar will appear.
- Select the date the exception occurred.
- Click Next.
- This will present the following screen
- All fields marked with an asterisk (*) are mandatory.
- Date of Exception - Displays the Date of Exception selected
- Time - Enter the Time in which the Exception took place.
- Work Pattern - Select work pattern if it appears in dropdown, not mandatory
- Specialty – Select the Specialty for which department/Specialty you are associated to.
- Location – Select from the dropdown of options for where the exception occurred
- Primary Supervisor – Select the Supervisor who will action the Exception Report.
- Also Notify – Select from the dropdown of options a member of staff to be notified of this Exception Report creation.
- Nature of Exception– Select Education or Hours and Rest, or both.
- Education – If ticked you will see a list of options to select against this nature.
- Hours and Rest– If ticked you will see a list of options to tick against this nature.
- Select the Nature of Exception.
- When choosing Hours and Rest, additional hours are to be included within the Exception Report.
- Click into the specific Rate Period to add Hours - which time frame did the exception occur
- Click the No of hours within the dropdown of options
- Immediate Safety Concern – Tick the box an immediate Safety concern is to be raised. Please follow your own Trust procedures to raise Safety concerns, ticking this box will not start the procedure.
- Who has the Safety Concern been raised with - Document the name of the person the concern was raised with.
- Description of Exception – Type a description providing detail of the variance(s) from the staff members Work Pattern
- Action taken – Type a description, outlining the steps the staff member has taken to resolve matters before escalation (if any).
- Once you have completed the Exception report click Submit.
Once you have submitted the Exception it will be available to view in the Submitted Tab.
You can also view Agreed, Not Agreed and Fulfilled exceptions on the associated tabs.
Viewing Exceptions
- Click on Home icon and select View Exception Reports.
- This will present the following screen
- Click the Eye Icon to open the Exception Report.
- This will present the following screen
- Click Done to close the Exception Report Details.
