Firstly you will need to go to your 'Billing' page in the navigation bar. From here you are taken to your billing page, click on 'Pensions'.
Here is where all the sessions that have been worked at your practice are listed, with a link to download the pension form. If there is no pension form for a session it will say 'No Pension'.
Scroll through the list to find the session for which you require a pension form. Once you have found the session click 'Download' if you have downloaded the form previously or 'Download and Sign' if this is the first time you have downloaded this form. Your download will start automatically.
